Hello,
I have few questions regarding form i 765 and i 821 forms.

1. If we entered USA using non MRP passport and have since received MRP, which passport are we supposed to send? case when non MRP passport hasn't expired yet.

2. i 765 #16 What we are supposed to enter in parenthesis (a) (12) or (a) (19) or both and how?

3. If working under CPT, do we need to send CPT copy?

Any help will be greatly appreciated!

sent both passports, use c19 code for ead, include CPT copy as well

For no 2. what if you are an out of status F1?

#15  Current Immigration Status   Overstay   ( if your visa is expired, DON'T WRITE ILLEGAL 

I- 765 #16 Eligibility Category.

(a) (12) = meaning= Temporary Protected Status

(c) (19) =meaning= Temporary Protected Status Applicant.

Few questions:
1. Form i821 : part 2 -4 alien registration number :- as an f1 I believe we don't have it right ? So leave it blank?
2. For i821: part 2- 25 b : again as student I think it's DS on i94. What date do we put on that or leave it blank ?
3. For i821 : part 4 - 1 b. I have entered the United States on the following date and have resided in the states since that time :- do we put last date of arrival to states on it or the date when we first arrived to US? To make it more clear say I came to US in 2008 and went back in 2010 for a month vacation and came back again in 2010. So should I use 2010 or 2008 ?

4. Form I-764 : 16- eligibility category : I believe it's c19 but still want to make sure .

@zxcv Port of Entry is normally airport and you can see that in your I94 record as well. But if you entered by road driving, it doesnt have to be airport.
