 TPS-Attached I-94 document missing

Did anyone received new I-94 attached with the TPS extension approval document. The first time I received it attached with the TPS approval letter on its bottom, which is of course expired on December  .
But when I applied the extension, I haven't got I-94 attached with the TPS extension approval letter. I only received one page approval letter. Now, I need a valid I-94 for my TWIC card approval for my job. So making sure did they send the I-94 the second time with the extension approval letter/notice of action. or I lost the page. Please share
