 i693: vaccination requirement

hello friends,
wanted to know about i-693. I dont know what all vaccinations are required and I dont have any record of any vaccination. Medical reps saying it may cost >800/each which is pretty expensive for me and wife if I dont have any medical record.
I did couple of vaccination when I started my school but dont know exactly.
I live in VA. Can someone please give me some details where I can pay less and get the things done?

The required vaccines are Tdap, MMR, Varicella, Hepatitis A, Hepatitis B and Flu. If you take those at your primary care physician's clinic, your insurance should cover. If you you have taken but don't have any records, retaking them won't have any adverse effects. Then take that record to the USCIS approved doctor for the I-693 medical exam. Usually insurance pays 100% for immunizations. If you take those at the USCIS doctor, then the insurance might not cover.

Get your vaccines at your primary care and if you have insurance you will be fully covered. Your primary doc can give you a report of vaccines that you can take to USCIS doc. These are the vaccination that I had to take :
1. MMR
2. DTP
3. Influenza.

First go to USCIS approved doc and get the recommended list or vaccines and then take it to your primary care doc and get them.

If you don't have insurance then I am not sure how it works but there may be someone here who may be able to help.

I had mine done for $250 at:

Annandale Family Practice

It was accepted by the USCIS without any issues.

It cost me around 230 ( check up plus chest x-ray) for single person without insurance. Mine was Desi civil surgeon (USCIS approved in California). You can do other vaccinations with your healthcare provider. There should be some desi Civil surgeons in your area.

It was $250 for me including PPD test for TB and Syphilis RPR test. Vaccines mandatory includes tdap (additional $65), MMR. I did have papers from Nepal which helped. Do mention them as you received BCG vaccine as you are from Nepal, which can give false positive PPD test and you might have to get a chest x-ray. Try to obtain all your vaccination records from your school by calling them... that will be very helpful.
In Nepal, we have yellow/pink vaccination record which if you can get will be helpful in saving some money and painful needle pricks :/ . Without the records you might have to get a couple of shots (guestimate 3-4 shots, + 1 ppd, +1 blood draw).

After reading all your comments, I tried to gather the forms I had submitted to school and found out the below one which was sent by one of my fren in Nepal to me. At the top (Cropped), It has my university details and my details. At the end, the clinic details (from Nepal) is mentioned. Does this work? It was prepared and signed by a Dr. in Aug,2012
